Through our Family Wellness programs, our trained community health workers provide case management, education, clinical navigation and home visiting services to mothers, fathers, couples and children.

Our community health workers are available to meet in the home, at the clinic, by phone, or in other community settings so they can best address their client’s needs. We can help enroll participants in health insurance, connect them to community resources (such as food, childcare, or housing), and provide translation assistance.

Programs are offered in English, Spanish and Marshallese.

We offer a variety of educational opportunities for parents. We are currently enrolling fathers into our free parenting classes, and are able to provide one-on-one education to clients in any of our Family Wellness programs. Our Fatherhood parenting course is open to any father, whether you’re expecting a child soon, or have a child under 24.

Our trained staff make sure mothers get the care they need during and after pregnancy. We provide prenatal and postpartum support, help with family planning and goal setting, and provide  in-home wellness services. Home visiting services are for pregnant or postpartum mothers and includes breastfeeding support, prenatal and postpartum clinical home visits, injury prevention strategies, safe sleep education, and more.
