Volunteer Licensed Health Care Professional Agreement

UAMS North Street Mission Statement: The purpose of the UAMS NW Student Clinic is to deliver compassionate, evidence-based, student-led, interprofessional care to disadvantaged residents of Northwest Arkansas with foci on education, prevention and management of chronic disease. UAMS students will screen, evaluate, educate, treat, refer, and perform other functions as necessary based on individual need. We strive to improve the health, wellness and access to healthcare in the community. To be seen in the clinic, patients must initial waiver stating “I am aware that volunteer health care professionals rendering medical services are immune from civil suit.”
